There is a doctor exam in game, however it it closed at the moment. This is not how you apply for doctor. If you want to apply for doctor, be patient and wait for the exam to open.

Third, this forum thread is for the Hospital Secretary role, not Doctor. If you'd like to become a Doctor, you can take the exam in the Capital Hospital. Although, I won't guarantee that it is open.
